Chẩn đoán phân biệt với bệnh giun lươn
Chẩn đoán phân biệt bệnh giun lươn cần bao gồm các bệnh do ký sinh trùng và không do ký sinh trùng. Việc xem xét các loại KST khác cần dựa trên tiền sử du lịch gần đây và xa của bệnh nhân. Các bệnh không do ký sinh trùng cần xem xét bao gồm những bệnh sau:
- Phản ứng phản vệ;
- Viêm da tiếp xúc;
- Bệnh phổi tắc nghẽn mãn tính (ở bệnh nhân có triệu chứng hô hấp);
- Phản ứng thuốc;
- Viêm dạ dày ruột do BCAT hoặc do vi khuẩn;
- Erythema annulare centrifugum (dạng phản ứng da có ban đỏ hình tròn);
- Bệnh ban xuất huyết Henoch-Schönlein;
- Hội chứng kém hấp thu;
- Suy dinh dưỡng ;
- Viêm màng não;
- Các bệnh ký sinh trùng khác (như bệnh sán máng cấp tính);
- Bệnh loét dạ dày tá tràng;
- Viêm phổi;
- Viêm đa động mạch dạng nốt;
- Bệnh ghẻ;
- Nhiễm trùng huyết và/hoặc sốc nhiễm trùng;
- Bệnh Lupus ban đỏ hệ thống;
- Hội chứng tăng BCAT phổi thoáng qua;
- Viêm loét đại tràng;
- Nổi mày đay;
- Chảy máu đường tiêu hóa trên và dưới.
Sàng lọc đối tượng nguy cơ, giảm thiểu biến chứng
Mặc dù bệnh giun lươn không phải là bệnh đặc hữu ở nhiều vùng của Mỹ, nhưng các bác sĩ lâm sàng nên xem xét sàng lọc bệnh giun lươn ở một số nhóm dân cư nhất định, bao gồm những nhóm sau đây:
- Bệnh nhân được chẩn đoán nhiễm virus lymphotropic tế bào T ở người loại 1;
- Bệnh nhân đang mắc và điều trị bệnh ung thư máu;
- Người hiến tặng hoặc người nhận tạng;
- Bệnh nhân mắc chứng tăng bạch cầu ái toan không rõ nguyên nhân;
- Bệnh nhân có tiền sử đi du lịch đến các khu vực lưu hành bệnh giun lươn.
Điều trị & Quản lý ca bệnh
Theo truyền thống, điều trị bệnh giun lươn dựa trên thuốc thiabendazole (TBZ) liều 25 mg/kg/12 giờ trong 3 ngày liên tiếp, tuy nhiên do một số độc tính hoặc tác dụng phụ của thuốc ên gần đây đã tạm dừng không dùng tại một số quốc gia. Tuy nhiên, tác dụng phụ (suy nhược, đau thượng vị và mất phương hướng) thường gặp và ký sinh trùng không thể bị tiêu diệt trong tới 30% trường hợp. Ivermectin cho thấy ít tác dụng phụ hơn và hiệu quả hơn TBZ trong điều trị bệnh giun lươn. Hiệu quả của ivermectin (IVM) phụ thuộc vào từng phác đồ điều trị và tình trạng nhiễm bệnh. Đối với bệnh giun lươn mạn tính không biến chứng, ivermectin liều 200 μg/kg mỗi ngày một lần trong 2 ngày được khuyến cáo và có thể rất hiệu quả.
Một nghiên cứu cho thấy sau khi điều trị bằng ivermectin liều 200 μg/kg trong 2 ngày liên tiếp, 100% bệnh nhân đáp ứng tiêu chí khỏi bệnh; trong số những người được điều trị bằng ivermectin 200 μg/kg trong một ngày, chỉ có 77% đáp ứng tiêu chí khỏi bệnh. Tuy nhiên, có những nghiên cứu chứng minh hiệu quả tương đương (>93%) cho cả hai phác đồ. Ngoài ra, một thử nghiệm ưu thế ngẫu nhiên có đối chứng, đa trung tâm, nhãn mở, giai đoạn 3 cho thấy rằng nhiều liều IVM không cho thấy hiệu quả cao hơn và khả năng dung nạp kém hơn so với một liều duy nhất. Do đó, việc lựa chọn chiến lược sử dụng thuốc phù hợp nhất theo bối cảnh địa phương là điều bắt buộc.
Albendazole (ALB) đã được sử dụng rộng rãi để điều trị ký sinh trùng đường ruột từ năm 1982 và được coi là một liệu pháp thay thế. Liều khuyến cáo là 400 mg uống hai lần mỗi ngày trong 7 ngày. IVM mang lại nhiều ca khỏi bệnh hơn so với ALB. Điều trị kết hợp cả albendazole và ivermectin (ALB-IVM) sẽ cải thiện hiệu quả điều trị thuốc hàng loạt nhắm vào S. stercoralis.
Như trên đề cập, điều trị đầu tay cho bệnh giun lươn là ivermectin, một loại thuốc phổ rộng có dạng viên uống 3 mg hay 6 mg. Albendazole là một loại thuốc tẩy giun khác có thể được sử dụng ngoài chỉ định để điều trị bệnh giun lươn. Liều lượng điều trị bệnh giun lươn phụ thuộc vào loại thuốc được sử dụng và mức độ nghiêm trọng của bệnh. Các khuyến nghị điều trị cho bệnh giun lươn cấp tính và mãn tính, cũng như hội chứng nhiễm trùng nặng và bệnh giun lươn lan tỏa được tóm tắt trong Bảng 2 và 3 tương ứng.

Nên sử dụng liều thấp hơn cho bệnh nhân có cân nặng nhỏ hơn hoặc bằng 80 kg; xem thông tin kê đơn được FDA phê duyệt. Từ Trung tâm Kiểm soát và Phòng ngừa Dịch bệnh và thông tin kê đơn được FDA phê duyệt.

Đối với những bệnh nhân không thể dung nạp thuốc đường uống (chẳng hạn như những người bị tắc ruột, kém hấp thu hoặc tắc nghẽn), ivermectin có thể được dùng qua đường trực tràng. Trong trường hợp cả đường uống lẫn đường trực tràng đều không khả thi, việc điều trị bằng ivermectin thú y tiêm dưới da có thể được thực hiện thông qua miễn trừ Thuốc Mới Nghiên cứu từ Cục Quản lý Thực phẩm và Dược phẩm Mỹ. Tác dụng phụ liên quan đến điều trị và tương tác thuốc.
Ivermectin
Một số phản ứng bất lợi đã được ghi nhận liên quan đến việc sử dụng ivermectin, bao gồm các phản ứng sau: Đau bụng, chứng chán ăn, táo bón, tiêu chảy, chóng mặt, mệt mỏi, viêm gan, hạ huyết áp, buồn nôn và nôn mửa, ngứa, phát ban, buồn ngủ, hội chứng Stevens-Johnson, sự rung chuyển cảm giác, hoại tử biểu bì nhiễm độc, nổi mề đay và chóng mặt.
Các kết quả xét nghiệm bất thường có thể xảy ra sau 1 hoặc 2 liều ivermectin và có thể bao gồm sau đây: Số lượng bạch cầu giảm, nồng độ bilirubin tăng cao và men gan tăng cao. Mặc dù hiếm gặp, nhưng đã có báo cáo về tình trạng tăng tỷ lệ chuẩn hóa quốc tế ở những bệnh nhân được điều trị bằng ivermectin đồng thời cũng đang dùng warfarin.
Albendazole
Các tác dụng phụ liên quan đến albendazole thường nhẹ và tự khỏi mà không cần điều trị. Các phản ứng phụ có thể xảy ra liên quan đến albendazole bao gồm đau bụng, chóng mặt, sốt, đau đầu, buồn nôn và nôn mửa, rụng tóc (có thể hồi phục) và chóng mặt. Các phản ứng phụ nghiêm trọng hiếm gặp đối với albendazole bao gồm độc tính gan và ức chế tủy xương.
Albendazole có thể tương tác với một số loại thuốc, bao gồm loại thuốc cimetidine, dexamethasone, praziquantel và theophylline.
Theo dõi bệnh nhân sau điều trị bệnh giun lươn: Bệnh nhân có triệu chứng dai dẳng và kết quả xét nghiệm phân dương tính với Strongyloides sau điều trị nên được xét nghiệm phân theo dõi 2 đến 4 tuần một lần để xác nhận nhiễm trùng đã được loại bỏ. Điều trị lại là phù hợp đối với những bệnh nhân có mẫu phân dương tính sau lần điều trị đầu tiên. Albendazole và ivermectin có liên quan đến sự tăng tạm thời men gan. Việc theo dõi men gan và xét nghiệm công thức máu toàn phần (CTMTP) có phân tích bạch cầu có thể cần thiết đối với bệnh nhân sau khi điều trị bằng ivermectin hoặc albendazole.
Gần đây, một nghiên cứu đã chỉ ra rằng moxidectin, một loại thuốc được Cục Quản lý Thực phẩm và Dược phẩm Mỹ (US.FDA) phê duyệt để điều trị bệnh giun chỉ ở người, có thể là một lựa chọn thay thế an toàn và hiệu quả cho ivermectin trong điều trị nhiễm trùng S. stercoralis.
Ở những bệnh nhân suy giảm miễn dịch hoặc mắc hội chứng nhiễm giun lươn nặng, có thể cần thêm liều hoặc kết hợp thuốc. Việc điều trị bằng IVM nên tiếp tục cho đến khi mẫu phân hoặc đờm âm tính trong 2 tuần đối với bệnh lan rộng. Nếu có thể, nên xem xét giảm hoặc ngừng sử dụng thuốc ức chế miễn dịch , đồng thời cung cấp hỗ trợ dinh dưỡng, bù nước, thuốc giảm đau và kháng sinh. Ở những bệnh nhân bị nhiễm trùng nặng không thể dùng thuốc tẩy giun hoặc bị tắc ruột non có thể ngăn cản sự hấp thu qua đường ruột, các chế phẩm IVM tiêm dưới da và thụt tháo dành cho thú y đã được sử dụng.
Bệnh nhân nên được xét nghiệm phân 03 lần trong vòng 2-4 tuần sau điều trị và xét nghiệm huyết thanh học giun lươn sau 6 tháng điều trị có thể hữu ích để xác định khỏi bệnh.
Tiên lượng
Tiên lượng bệnh giun lươn phụ thuộc vào mức độ nghiêm trọng của nhiễm trùng va có hay không bệnh lý nền đang có. Tỷ lệ tử vong đối với bệnh giun lươn không được điều trị có thể lên tới 90% ở những bệnh nhân mắc bệnh giun lươn nặng, những người phát triển hội chứng nhiễm trùng lan rộng và bệnh giun lươn toàn thân hoặc đang suy giảm miễn dịch và đang nằm ở khu hồi sức tích cực với nhiễm vi khuẩn đa kháng thuốc.
Phòng ngừa và Kiểm soát
Mặc dù tính chất gây tử vong của nhiễm trùng nặng và các nghiên cứu đã làm tăng sự chú ý đến nhiễm trùng S. stercoralis, nhưng các kế hoạch hành động có hệ thống vẫn còn chậm trễ. Việc thực hiện các chương trình kiểm soát nhiễm trùng S. stercoralis là một mục tiêu trong Lộ trình đến năm 2030 của Tổ chức Y tế Thế giới. Việc tăng cường thông tin, giáo dục và truyền thông sẽ rất cần thiết để nâng cao hiểu biết của cộng đồng về bệnh giun lươn và việc phòng ngừa, điều trị và kiểm soát bệnh này.
Giáo dục sức khỏe nhấn mạnh vệ sinh cá nhân ở các khu vực lưu hành bệnh nên được cung cấp cho bệnh nhân, những người có nguy cơ và dân chúng nói chung. Ở các cộng đồng nông thôn và vùng sâu vùng xa ở các vùng nhiệt đới, việc tiếp cận với hệ thống vệ sinh sạch sẽ, nước sạch và giày dép là điều cơ bản để ngăn ngừa bệnh giun lươn.
Xét nghiệm bệnh giun lươn rất quan trọng ở những bệnh nhân đến từ các khu vực lưu hành bệnh, các cộng đồng nông thôn và vùng sâu vùng xa và những người bị suy giảm miễn dịch. Đối với trẻ em và người lớn có nguy cơ, cần nỗ lực để loại bỏ hoặc giảm nhiễm trùng S. stercoralis bằng cách phát hiện sớm và điều trị ngay lập tức. Nên áp dụng biện pháp cách ly tiếp xúc đối với bệnh nhân nhập viện bị nhiễm trùng nặng và tất cả thành viên trong gia đình nên được xét nghiệm nhiễm trùng S. stercoralis. Bệnh nhân trải qua ghép tạng hoặc đang dùng thuốc ức chế miễn dịch nên được sàng lọc nhiễm trùng S. stercoralis. Ngoài ra, cần xem xét bệnh giun lươn ở bệnh nhân tăng BCAT.
Bệnh giun lươn có thể được phòng ngừa bằng cách tránh các tình huống làm tăng nguy cơ tiếp xúc với giun lươn. Các chiến lược để tránh tiếp xúc với giun lươn bao gồm mang giày dép khi ra ngoài và mang găng tay khi làm vườn và làm cây cảnh, tránh tiếp xúc với nguồn nước thải. Bệnh giun lươn không phổ biến ở các quốc gia có hệ thống vệ sinh và xử lý chất thải tốt.
Trong một số nghiên cứu, việc chẩn đoán chậm trễ, kiến thức không đầy đủ, thiếu giao tiếp và thiếu theo dõi từ các chuyên gia y tế được mô tả là những vấn đề nghiêm trọng. Các chuyên gia y tế cần được yêu cầu có thông tin và giáo dục chi tiết về bệnh giun lươn. Điều quan trọng là phải nhận ra rằng nhiễm S. stercoralis có thể tồn tại trong nhiều năm với các triệu chứng và dấu hiệu không đặc hiệu. Thiếu nhận thức về nhiễm trùng mãn tính ở bệnh nhân làm tăng nguy cơ lây nhiễm trong cộng đồng của bệnh nhân và làm giảm nhận thức của các chuyên gia y tế và cộng đồng về sự cần thiết phải loại bỏ sự lây nhiễm trong cộng đồng. Do đó, các chuyên gia nên thiết lập ngưỡng tỷ lệ hiện mắc để xác định S. stercoralis là một vấn đề sức khỏe cộng đồng và đề xuất các chiến lược kiểm soát, bao gồm cả các phác đồ điều trị hàng loạt.
Ngoài ra, việc kết nối mạng lưới là rất quan trọng giữa các nhà nghiên cứu quan tâm đến S. stercoralis để cải thiện sự phối hợp và tối ưu hóa nguồn lực. Hơn nữa, điều quan trọng là phải thiết lập các sáng kiến xét nghiệm và điều trị trong cộng đồng. Các giao thức báo cáo về S. stercoralis và bệnh giun lươn có thể được thiết lập giữa các hệ thống chăm sóc sức khỏe và cộng đồng. Các nghiên cứu về tỷ lệ mắc bệnh giun, sán đường ruột cũng nên nhắm vào S. stercoralis, và điều quan trọng là phải phát triển các phương pháp chẩn đoán nhạy và đặc hiệu hơn. Các nhà tài trợ và các cơ quan tài trợ không nên bỏ qua S. stercoralis.
Đối với các quốc gia/ khu vực có tỷ lệ mắc bệnh cao, nên cung cấp ivermectin để điều trị hàng loạt. Tuy nhiên, việc thực hiện các biện pháp như vậy bị cản trở bởi sự tập trung vào virus gây suy giảm miễn dịch ở người (HIV/AIDS), sốt rét, bệnh lao, các bệnh bị lãng quên khác và chiến tranh.
Kết luận
Nhiễm trùng do S. stercoralis rất phổ biến ở vùng nhiệt đới và cận nhiệt đới, đặc biệt là ở cộng đồng nông thôn, vùng sâu vùng xa với điều kiện vệ sinh kém. Các bác sĩ lâm sàng cần xem xét khả năng nhiễm S. stercoralis ở những bệnh nhân SGMD, đang sử dụng thuốc gây độc tế bào, mắc các bệnh SGMD và ghép tạng.
Cần có sự tham gia của các chuyên gia y tế, cam kết tài chính và các chính sách để tăng cường quản lý nhiễm trùng S. stercoralis. Trong thập kỷ qua, ngày càng nhiều nỗ lực đã được dành cho nghiên cứu vaccine, các chiến lược điều trị hiệu quả và chẩn đoán được cải tiến. Cần bổ sung các biện pháp khác để nâng cao chẩn đoán và điều trị.
Cần xây dựng và thực hiện các chiến lược kiểm soát mới, bao gồm tăng cường thông tin, giáo dục, truyền thông, quản lý bệnh nhân, giám sát dịch tễ học, cải thiện môi trường, vệ sinh tốt hơn và nước sạch và xây dựng thông qua sự hợp tác liên ngành.
