Bệnh do xoắn khuẩn Leptospira spp. (Leptospirosis) là bệnh truyền nhiễm cấp tính, do xoắn khuẩn Leptospira spp. gây ra, lây truyền chủ yếu qua đường da, niêm mạc.Đặc điểm lâm sàng là hội chứng nhiễm khuẩn-nhiễm độc toàn thân và hội chứng tổn thương gan, thận. Đây là bệnh của động vật lan truyền sang người, có ổ bệnh thiên nhiên. Nguồn bệnh gây ra từ động vật, gia súc như cừu, dê, lợn, chó, mèo. Ngoài ra, ở nhiều loại động vật hoang dã như gấu, báo, chuột… Người mắc bệnh ngẫu nhiên chứ không phải là nguồn bệnh.

Tuy nhiên, một số tác giả cho rằng, có sự lây truyền từ người sang người do đào thải xoắn khuẩn qua đường nước tiểu của người bệnh. Lây qua đường tiêu hóa như qua thức ăn, nước uống không đun sôi, nấu chín, bị ô nhiễm. Cá biệt là lây qua đường hô hấp do hít phải các giọt nước nhiễm khuẩn ở dạng khí dung.

Mọi lứa tuổi, mọi giới đều có thể bị mắc bệnh. Tuy nhiên, bệnh mang tính chất nghề nghiệp hay gặp ở nông dân lội ruộng, người chăn nuôi súc vật, bộ đội luyện tập nơi bùn lầy nước đọng. Hiện nay, bệnh do Leptospira spp. được xếp vào nhóm bệnh nghề nghiệp được bảo hiểm. Về miễn dịch, sau khi bị bệnh để lại miễn dịch bền vững nhưng chỉ với type huyết thanh mắc bệnh. Do vậy, vẫn có thể bị lại với type khác. Dịch thường tản phát ở vùng có ổ dịch lưu hành, có khi gây dịch lớn, hay xảy ra vào mùa hè thu. Ở nước ta có nhiều ổ dịch ở các tỉnh miền núi phía Bắc và khu vực miền Trung-Tây Nguyên.

 Sau khi qua da và niêm mạc, vi khuẩn Leptospira vào máu gây nhiễm khuẩn huyết, kéo dài khoảng 5-7 ngày, tương ứng với giai đoạn khởi phát. Sau đó Leptospira spp. khu trú vào các tạng gan, thận, màng não, tim, phổi, thượng thận và gây tổn thương các tạng này. Giai đoạn này kéo dài 7-8 ngày, tương ứng với thời kỳ toàn phát. Thường từ ngày thứ 8 của bệnh, xoắn khuẩn thải ra ngoài qua đường nước tiểu. Tổn thương gan trong Leptospira spp. gây triệu chứng vàng da. Nguyên nhân do tổn thương mạch máu nuôi dưỡng tế bào gan dẫn đến hoại tử tế bào và do độc tố xoắn khuẩn gây hủy hồng cầu.Tổn thương thận, chủ yếu là tổn thương ống thận gây thiểu vô niệu, ure và creatinin máu tăng là nguyên nhân chính gây tử vong. Nguyên nhân gây tổn thương ống thận là do thiếu oxy máu và do tác động trực tiếp của nội độc tố Leptospira spp. Bệnh nhân có thể xuất huyết do nguyên nhân độc tố làm tổn thương thành mạch và một phần do đông máu nội mạch.     

Giai đoạn nhiễm khuẩn huyết, thường kéo dài 4-9 ngày biểu hiện với sốt cao đột ngột 39-40⁰C, rét run, sốt liên tục hoặc dao động kèm theo mạch nhanh, huyết áp dao động.Mệt nhiều, đau đầu, nhức mắt, buồn nôn, nôn. Những trường hợp nặng có biểu hiện li bì, vật vã mê sảng.Da niêm mạc xung huyết, dãn mạch rõ, mắt đỏ, nhiều tia máu giống như mắt cá chầy, có thể có xuất huyết dưới kết mạc. Niêm mạc mũi xung huyết hay chảy máu cam. Có thể xuất hiện ban dát sần cuối tuần đầu của bệnh.Đau cơ thường dữ dội, tự nhiên và tăng khi sờ nắn, nhất là cơ bắp chân, cơ đùi, cơ thẳng bụng, cơ gáy. Đau làm cho bệnh nhân đi lại khó khăn, không dám thở sâu. Thường đánh dấu bằng cơn đa niệu và nhiệt độ giảm dần. Các triệu chứng toàn thân giảm, bệnh nhân hồi phục. Một số trường hợp tái phát, sốt lại nhưng nhẹ hơn.

Bệnh có thể xuất hiện biến chứng như suy thận cấp nặng dễ gây tử vong, biểu hiện vô niệu, ure máu, creatinin máu tăng cao, bệnh nhân hôn mê do ure máu cao. Tim mạch có thể viêm cơ tim, choáng, truỵ tim mạch. Xuất huyết ồ ạt các phủ tạng gây thiếu máu cấp, có thể có đông máu rải rác nội mạch. Phù phổi cấp, ngoài ra còn có thể gặp: liệt, viêm mống mắt thể mi, viêm thần kinh thị giác, gây mù.

Trên thực hành lâm sàng có hội chứng nhiễm khuẩn, nhiễm độc với đau cơ tự nhiên, tăng lên khi sờ nắn, hội chứng tổn thương gan, thận. Xét nghiệm đặc hiệu soi trực tiếpdưới kính hiển vi nền đen, bệnh phẩm lấy từ máu trong 5 ngày đầu của bệnh, dịch não tủy, nước tiểu ly tâm có thể thấy xoắn khuẩn di động nuôi cấy ở môi trường đặc hiệu (Terkich) hoặc tiêm truyền cho chuột lang.

Chẩn đoán huyết thanh có phản ứng ngưng kết tan Martin-Pettit, làm hai lần cách nhau 7 ngày. Phản ứng dương tính khi huyết thanh lần hai có hiệu giá tăng gấp 4 lần huyết thanh lần 1 hoặc làm 1 lần hiệu giá kháng thể cao trên 1/1000. Phản ứng miễn dịch huỳnh quang cho kết quả nhanh. Phản ứng ELISA nhạy, đặc hiệu. Chẩn đoán phân biệt viêm gan virut và viêm gan virut diễn biến: đều có sốt, vàng da nhưng trong viêm gan virut diễn biến có hai giai đoạn tiền hoàng đản và hoàng đản rõ rệt, khi vàng da thì hết sốt. Triệu chứng xuất huyết và suy thận chỉ gặp ở trường hợp nặng không có triệu chứng đau cơ và hội chứng màng não. Xét nghiệm men SGOT, SGPT tăng cao, ure máu bình thường hoặc giảm. Bạch cầu bình thường, tốc độ máu lắng không tăng. Da niêm mạc xung huyết, đau cơ, nhưng trong Dengue xuất huyết không có vàng da, đau cơ nhưng khi bóp vào thì đỡ đau. Xét nghiệm: Bạch cầu bình thường hoặc giảm, máu lắng bình thường, ure không tăng, tiểu cầu giảm, Hematocrit tăng, phản ứng HI(+).

Nguyên tắc điều trị: Phòng chống suy thận bằng cách bù đủ nước điện giải, sử dụng thuốc lợi tiểu khi có biểu hiện suy thận. Sử dụng kháng sinh đặc hiệu, những trường hợp bệnh nhẹ có thể sử dụng doxycyclin 200mg/ngày. Đối với bệnh nhân phải nhập viện nên sử dụng penicillin đường tĩnh mạnh. Penicillin có thể cắt sốt 4 - 6 giờ sau khi dùng thuốc và giảm sự phát triển của Leptospira spp. Thời gian sử dụng kháng sinh từ 5 -7 ngày. Các kháng sinh có thể thay thế là ampicilline, amoxicilline, tetracycline, doxycycline, erythromycine, streptomycine và cephalosporine...

Về phòng bệnh, đối với nguồn bệnh cần diệt chuột, phát hiện sớm và điều trị những động vật nuôi mắc bệnh hoặc mang mầm bệnh. Điều trị bệnh nhân và khử trùng ổ dịch.Bảo vệ tốt nguồn nước, thực phẩm, những vùng đầm lầy phải khai thông cống rãnh. Những người làm việc liên quan đến ổ dịch cần được trang bị quần áo bảo hộ (găng, ủng, bao tay), tránh xây sát da chân tay. Sử dụng vaccin sống giảm độc lực. Tuy vậy, việc phòng bệnh chưa được áp dụng rộng rãi và phải biết rõ type huyết thanh gây bệnh và thường chỉ dùng cho những đối tượng dễ mắc như: nhân viên thú y, bộ đội đi vào ổ dịch.
