Sự chấp thuận của cơ quan Quản lý Thực dược phẩm Mỹ về thuốc sốt rét Tafenoquine

Tin vui trong chuyên ngành dược sốt rét: Có thể tiến tới loại trừ sốt rét sớm hơn

Theo thông tin từ giáo sư Dennis Shanks, Viện trưởng Viện các bệnh truyền nhiễm và sốt rét Úc (Australian Defence Force Malaria and Infectious Disease Institute-ADFMIDI), trước đây là Viện Sốt rét Lục quân Úc, trực thuộc Bộ Quốc phòng Úc cho biết ngày hôm qua 22/7/2018 rằng loại thuốc Tafenoquine-một loại thuốc 8-aminoquinolein đã được Cơ quan Quản lý Thực dược phẩm Mỹ (US. FDA) chấp thuận và từ đây có thể Tafenoquine tiếp tục được các cơ quan chức năng cũng như các Chương trình Quốc gia Phòng chống sốt rét cân nhắc thay thế primaquin phosphate (PQ) bằng Tafenoquine với điều trị ngắn ngày (1 ngày 1 liều - single dose/one day treatment) để đạt được sự tuân thủ và chấp thuận của bệnh nhân sốt rét do P. vivax khi điều trị tận gốc (Tafenoquine has been approved by the US FDA for registration as a for the radical cure of vivax malaria), nhất là khi họ đã xuất viện về nhà và tiếp tục liệu trình.

Đây quả là một tin rất vui và vô cùng quan trọng đối với Viện các bệnh truyền nhiễm và sốt rét Úc với vai trò là một viện nghiên cứu chính về thuốc tafenoquine đã 20 năm về khâu điều trị tận gốc sốt rét do P. vivax (radical cure of vivax malaria) và dự phòng sốt rét đối với các loài KSTSR (prophylactic agent against all Plasmodium species). Đây cũng là Viện nghiên cứu duy nhất trên thế giới tiến hành thử nghiệm pha III về thuốc tafenoquine như một thuốc dự phòng cả P. falciparumP. vivax. Viện nghiên cứu sốt rét của lục quân Úc đã gần 20 năm hợp tác nghiên cứu với Viện Sốt rét Ký sinh trùng-Côn trùng Quy Nhơn và Viện Y học dự phòng Quân đội của Việt Nam trong nghiên cứu về lĩnh vực thuốc sốt rét mới và phác đồ phối hợp cũng như phòng chống vector tại Ninh Thuận, Quảng Bình, Gia Lai, Đăk Nông. Các kết quả nghiên cứu đã mang lại thành công thiết thực và có ý nghĩa trong việc chọn lựa thuốc sốt rét tại Việt Nam, thay đổi chính sách thuốc theo từng giai đoạn, giúp giảm gánh nặng sốt rét và đạt được sự tuân thủ dùng thuốc của bệnh nhân vì có sự giám sát chặt chẽ trong nghiên cứu

Tafenoquine là một loại thuốc thuộc nhóm thuốc sốt rét 8-aminoquinolin sản xuất bởi công ty GlaxoSmithKline sau khi đánh giá có tác dụng như một liệu pháp điều trị tiềm năng cho căn bệnh sốt rét gần 25 năm trước cũng như ngăn ngừa sốt rét do thuốc này diệt thể ngủ và chống tái phát xa cho P. vivax. Chỉ định điều trị cho thuốc Tafenoquin là diệt thể ngủ cả Plasmodium vivax Plasmodium ovale vì hai loài ký sinh trùng sót rét này có thể gây tái phát xa trong máu sau khi đã hoàn tất liệu trình thuocs diệt thể vô tính thành công.

Thuốc giờ đây được chỉ định chỉ có duy nhất thay cho liệu pháp primaquine 14 ngày, nhưng thuốc hiện vẫn đã và đang hoàn tất các giai đoạn nghiên cứu thử nghiệm chứ chưa đưa ra sử dụng rộng rãi. Lợi điểm chính của thuốc Tafenoquine là thời gian bán hủy dài (2-3 tuần), nên điều trị liều duy nhất có thể đủ nồng độ thuốc theo liều dược lý làm sạch thể ngủ. Tương tự thuốc primaquine phosphate, thì Tafenoquine cũng có thể gây tán huyết trên những bệnh nhân thiếu men G6PD. Ngoài ra, do thời gian bán hủy kéo dài của Tafenoquine nên cần sự chăm sóc đặc biệt để đảm bảo những cá nhân có thiếu men G6PD nghiêm trọng không nên dùng thuốc. Liều thuốc Tafenoquine chưa xác định một cách đầy đủ, nhưng điều trị sốt rét do P. vivax với một liều 800 mg trong 3 ngày đã từng dùng.

Điều trị tiệt căn sốt rét do P. vivaxP. vivax đang gặp trở ngại do kháng primaquin

Hiện tại chloroquine (CQ) và primaquin phosphate (PQ) là thuốc dùng để điều trị thể vô tính và diệt tận gốc ngăn ngừa tái hoạt thể ngủ của của ký sinh trùng sốt rét Plasmodium vivax (P. vivax) tác động trên các thể ngoại hồng cầu, phá hủy các thể ngủ tiềm tàng nên chúng có khả năng ngăn ngừa P. vivax tái phát. Liệu pháp PQ hiện hành có thể làm tăng hoạt tính của CQ chống lại sự kháng thuốc của KSTSR P. vivax ở giai đoạn trong hồng cầu (Pukrittayakamee và cs., 1994; Baird và cs., 1995).

Dù nhiều báo cáo các vùng địa lý khác nhau thấy P. vivax kháng PQ nhất là tại Indonesia, song bằng chứng tin cậy thì chưa thuyết phục vì do nhiều yếu tố can thiệp như điều trị không có giám sát, dung nạp từ KST, khả năng tái nhiễm.

Tái phát có thể xảy ra sớm hoặc muộn (16 ngày đến 3 năm) kể từ khi lần nhiễm SR đầu tiên, ngay cả khi đã điều trị đủ liều. Do đó, các thử nghiệm lâm sàng nếu cho rằng kháng PQ thì xác định là lam máu vẫn dương tính với P. vivax sau khi đã điều trị đủ liều chuẩn và có điều trị PQ đủ liệu trình (14 ngày theo khuyến cáo của Tổ chức Y tế thế giới từ gần 30 năm qua) trong suốt quá trình theo dõi. Điều này sẽ khó khả thi vì sự chấp thuận của đối tượng nghiên cứu cũng như bệnh nhân điều trị và nhận điều trị khi họ đã không còn sốt nữa, nhất là bệnh nhân ở các vùng sâu vùng xa và nhóm dân tộc thiểu số có sự tuân thủ không đầy đủ.

Tafenoquine sẽ thay thế primaquine trong điều trị bệnh nhân P. vivax?

Theo thông tin từ giáo sư Ric Price thuộc Trung tâm nghiên cứu y học nhiệt đới của Đại học Oxford, Anh cho thấy 40% dân số thế giới có nguy cơ nhiễm ký sinh trùng Plasmodium vivax. Không như P. falciparum, loài P. vivax có thể ngủ và bị "đánh thức dậy" trong nhu mô gan sau vài tuần đến vài tháng, thậm chí vài năm kể từ khi có triệu chứng nhiễm đầu tiên đã điều trị khỏi. Các tình trạng nhiễm trùng tái phát này đã cho phép ký sinh trùng sống sót trong thời gian dài trong điều kiện muỗi ít (như các tháng mùa đông hay nhiệt độ ôn đới). Khả năng tồn tại của thể ngủ như vậy đã khiến cho sốt rét do P. vivax sẽ khó loại trừ hơn so với sốt rét P. falciparum.

Tại nhiều nơi có can thiệp của chương trình PCSR tích cực đang làm giảm tỷ lệ mắc sốt rét do P. falciparum nhưng đối với P. vivax thì chưa, điều này đang trở thành điểm nổi bật về sốt rét ở các quốc gia châu Phi. Tại nhiều vùng, P. vivax tái phát là một nguyên nhân chính của gánh nặng bệnh tật ở trẻ em nhỏ, dẫn đến nhiều cơn sốt chồng cơn, dẫn đến thiếu máu và dễ mắc các bệnh nhiếm trùng khác đi kèm theo.

Hầu hết các thuốc sốt rétcó đích tác dụng trên ký sinh trùng cụ thể vào các giai đoạn, nhưng hầu như không có thuốc nào trong số chúng có tác động lên thể ngủ trong gan của P. vivax. Hơn 60 năm qua, điều trị tiệt căn P. vivax là dựa vào thuốc chloroquine để diệt thể vô tính trong máu và thuốc primaquine loại bỏ thể ngủ. Tuy nhiên, primaquine (PQ) đòi hỏi liệu trình dài 14 ngày và vì bệnh nhân thường chỉ dùng một vài ngày rồi bỏ, nên họ khó có thể tiếp tục hoàn tất liệu trình đầy đủ của PQ.

Tafenoquine là một thuốc có tác dụng kéo dài cùng nhóm với PQ. Nó đã suy giảm trong quá trình phát triển 20 năm qua, nhưng gần đây quay trở lại với các ưu thế trong hợp chất vì nó có tiềm năng điều trị thể ngủ của P. vivax với liều duy nhất. Trong một nghiên cứu gần đây ở đa trung tâm ấn bản trên tạp chí The Lancet cho biết liều duy nhất 300mg Tafenoquine làm giảm nguy cơ tái xuất hiện ký sinh trùng trong vòng 6 tháng sau điều trị đến 10% so với 60% ở nhóm chỉ dùng đơn thuần chloroquin. Các nghiên cứu tiếp theo giờ đây đang điều tra thêm về vai trò của thuốc Tafenoquine độ an toàn và hiệu quả của thuốc như PQ.

Nếu các thử nghiệm lâm sàng tiếp theo có thể xác định tính an toàn và so sánh hiệu lực với các lựa chọn điều trị hiện nay và thuốc có thể triển khai dùng rộng rãi, thì liều dùng duy nhất của tafenoquine sẽ có tiềm năng điều trị hiệu quả và tiệt căn cho sốt rét P. vivax và sẽ là công cụ quan trọng trong cuộc chiến loại trừ sốt rét.

Điểm mới trong nghiên cứu P. vivax thời gian qua về hiệu lực thuốc PQ cũng cho thấy có tỷ lệ kháng của P. vivax. Do đó, nhiều thử nghiệm lâm sàng một thuốc mới như Tafenoquine thuộc nhóm 8-aminoquinolein, một đồng phân với PQ, có công thức phân tử 8-(4–amino-1-methylbutyl) amino-2,6-dimethoxhy-4 methyl-5-(3-trifluoromethylphenoxy) quinoline succinate), thuốc tác động thông qua cơ chế tích tụ thuốc trong khoang không bào của KST, gây khử động heme, ức chế hình thành hemozoine.

Như thuốc chloroquine đã từng dùng trên thế giới trên 60 năm qua, thì thuốc primaquine phosphate cùng đã dùng trên thế giới từng ấy thời gian và thuốc Tafenoquine ra đời vào khoảng những năm 1980.

Thử nghiệm điều trị P. vivax liệu trình 3 ngày với tổng liều 800mg thay vì CQ liều 1500mg liệu trình 3 ngày và PQ 420mg trong 14 ngày. Kết quả bước đầu của hãng dược phẩm Glaxo Smith Kline (GSK) và Tổ chức nghiên cứu sốt rét Medicines for Malaria Venture (MMV) trong pha I và II cho hiệu quả vừa đáp ứng lâm sàng tốt, vừa không thấy tái phát sớm cũng như tái phát xa. Các nghiên cứu in vitro, cũng chỉ ra hoạt tính chống lại thoi trùng và giao bào nên có tiềm năng cắt lan truyền bệnh.

Trong các nghiên cứu tiền lâm sàng, Tafenoquine cho thấy nồng độ trong máu và trong mô cao gấp 3-67 lần so với thuốc PQ và các nghiên cứu in vitro có tác động cộng lực với hoạt tính của CQ và artemisinin, nên khi phối hợp sẽ cho hiệu quả tối ưu.

Các nghiên cứu tiếp theo phase III chuẩn bị triển khai vào tháng 5.2014 tại đa quốc gia, trong đó có Việt Nam và pha này tiếp tục đánh giá trên nhóm phụ nữ mang thai và đối với nhóm đối tượng thiếu men G6PD. US FDA đã chính thức công bố hiệu quả và vai trò quan trọng của tafenoquine

Sau 20 năm nghiên cứu, giờ đây các nhà khoa học đã mô tả tafenoquine như là một "thành tựu phi thường".Các nhà chức trách Mỹ cho biết thông tin về thuốc Tafenoquine để điều trị bệnh sốt rét đã được chấp thuận.Loại thuốc này đặc biệt dành để điều trị chủng KSTSR gây tái phátPlasmodium vivaxmà đã từng gây bệnh cho 8,5 triệu người mỗi năm.Chủng sốt rét này là một thách thức trong việc loại bỏ nó không hề dễ dàng vì nó có thể không hoạt động trong gan trong nhiều năm trước khi "tái thức tỉnh" nhiều lần.Các nhà quản lý sốt rét trên toàn cầu giờ đây sẽ xem xét liệu loại thuốc Tafenoquine có thể khuyến cáo dùng cho các quần thể dân hay không.

Sốt rét tái phátdo loại ký sinh trùng sốt rét phổ biến nhất ở khu vực ngoài vùng cận sa mạc Sahara, châu Phi.Đối tượng có nguy cơ là trẻ em nhỏ, bị mắc nhiều cơn sốt rét từ một vết đốt duy nhất, chúng khi mắc bệnh có thể dẫn đến bỏ học và trở nên yếu hơn mỗi lần bị bệnh. Các bệnh nhân có thể hoạt động như những"ổ chứa mầm bệnh" không mong muốn của căn bệnh này bởi vì khi KSTSR tỉnhgiấc trong cơ thể của họ, một con muỗi có thể mang KSTSR đó cho người khác.

Điều này có thể gây ra khó khăn cho việc loại trừ sốt rét trên toàn thế giới. Giờ đây Cục Quản lý Thực phẩm và Dược phẩm Mỹ (US. Food and Drug Administration - FDA) ở Mỹ đã chấp thuận cho Tafenoquine, một loại thuốc có thể làm cho ký sinh trùng ra khỏi nơi ẩn náu của nó trong tế bào gan và ngăn chặnngười bị nhiễm mắc lại nó.Nó có thể được dùng cùng với một loại thuốc khác để điều trị nhiễm sốt rét ngay lập tức.

Từ rất lâu, loại thuốc có thể được sử dụng để loại bỏ sốt rét tiềm ẩn trong nhu mô gan là primaquine phosphate, nhưng không giống như liều duy nhất 1 ngày 1 liều của Tafenoquine, Primaquine cần phải dùng kéo dài đến 14 ngày. Điểu này là một bất lợi do bệnh nhân không tuân thủ dùng thuốc vì nhiều bệnh nhân cảm thấy đỡ hơn chỉ sau vài ngày dùng thuốc đã ngừng uống, nên cho phép ký sinh trùng thức dậy vào một ngày nào sau đó.

Cần thận trọng khi dùng Tafenoquine trong điều trị sốt rét

Cơ quan FDA cho biết thuốc Tafenoquine có hiệu quả và phê chuẩn nó để sử dụng tại Mỹ nhưng cũng chỉ ra rằng có những tác dụng phụ quan trọng cần lưu ý.Ví dụ những người có vấn đề về bệnh lý bất thường enzyme như thiếu hụt men G6PD, không nên dùng thuốc Tafenoquine vì nó có thể gây thiếu máu trầm trọng.

Do đó, Cơ quan quản lý khuyến cáo mọi người nên xét nghiệm có thiếu men này hay không trước khi sử dụng thuốc và điều này có thể gây ra vấn đề nơi bệnh sốt rét phổ biến. Ngoài ra, còn có những lo ngại rằng ở liều cao hơn, nó có thể là một vấn đề cho những người bị bệnh tâm thần.

Nhưng bất chấp những cảnh báo này có hy vọng thuốc này cùng với màn ngủ tẩm hóa chất các biện pháp phòng ngừa khác, sẽ giúp giảm số lượng bệnh sốt rét do P. vivax trên thế giới.Giáo sư Ric Price, thuộc Đại học Oxford nói với BBC rằng khả năng loại bỏ ký sinh trùng trong gan chỉ bằng một liều Tafenoquine duy nhất là một thành tựu vĩ đại và quan trọng, trong tâm trí của ông nó là một trong những tiến bộ đáng kể nhất trong điều trị bệnh sốt rét trong vòng 60 năm qua.

Trong khi đó, TS. Hal Barron, Chủ tịch nghiên cứu và phát triển của tập đoàn dược phẩm GSK cho biết đây là một cột mốc quan trọng đối với những người bị nhiễmchủng sốt rét tái phát này.Cùng với các đối tác, liên doanh thuốc dành cho sốt rét họ tin rằng Krintafel sẽ là một loại thuốc quan trọng cho bệnh nhân sốt rét và góp phần vào nỗ lực liên tục để loại trừ căn bệnh này.

Tafenonquine còn gọi là tên WR 238605 hay SB-252263- là loại thuốc đã tồn tại từ những năm 1970 nhưng nghiên cứu với công ty thuốc chữa sốt rét (MMV), GSK đã tái sử dụng loại thuốc này để nó có thể được sử dụng để loại bỏ ký sinh trùng sốt rét trong gan. Bước tiếp theo sẽ là thuốc được đánh giá bởi các nhà quản lý ở những quốc gia nơi mà chủng sốt rét P. vivax là một vấn đề y tế quan trọng.
