Những ấn phẩm khoa học mới về nghiên cứu sốt rét

Cho đến nay sốt rét vẫn là một trong những căn bệnh gây tử vong hàng đầu trong số các bệnh truyền nhiễm trên toàn thế giới, nên các kinh phí và nguồn lực hiện đã và đang tiếp tục đầu tư nghiên cứu cả cơ bản và cả nghiên cứu chuyen sâu ứng dụng vào lĩnh vực sốt rét, cả về sinh bệnh học, chẩn đoán, điều trị và thuốc sốt rét mới. Ban Biên tập Website xin chuyển đến quý bạn đồng nghiệp những ấn phẩm mới về nghiên cứu thời gian gần đây.

Thành quả của Chương trình quốc gia phòng chống sốt rét (PCSR) đạt được nhờ vào sự can thiệp tổng hòa các biện pháp phòng chống vector, vai trò điều tra dịch tễ chỉ đạo, phát hiện sớm và điều trị kịp thời, nghĩa là không thể không kể đến hiệu quả thuốc sốt rét. Giám sát thường quy và chặt chẽ tình hình kháng thuốc để trên cơ sở đó đề xuất các phác đồ điều trị sốt rét có hiệu quả và quản lý ca bệnh kịp thời là một trong những điểm chính yếu làm giảm sốt rét. Từ năm 1990, thuốc chống sốt rét artemisinine và dẫn chất artesunate được tiến hành thử nghiệm lâm sàng và chính thức đưa vào sử dụng rộng rãi trong Chương trình PCSR ở nước ta đã có vai trò quan trọng trong trong việc hạ thấp tỷ lệ bệnh và tỷ lệ tử vong do sốt rét. Mặc dù với liệu trình 5 ngày, tỷ lệ khỏi bệnh do sốt rét P. falciparum có thể đến 80-90%, nhưng nhược điểm lớn nhất của artemisinine/artesunate điều trị đơn thuần là tỷ lệ tái phát sớm sau điều trị quá cao (35-50%) hoặc 7-50%; để khắc phục nhược điểm này, nhiều phác đồ điều trị sốt rét phối hợp có gốc artemisinine hoặc dẫn suất (ACTs_Artemisinine-based Combination Therapies) đã được nghiên cứu sử dụng ở nhiều nước trên thế giới, trong đó có Việt Nam như thuốc như thuốc CV-8 [Dihydroartemisinie + Piperaquine phosphate + Primaquine phosphate + Trimethoprim], Artekin, Arterakine, CV Artecan [Dihydroartemisinie 40mg + Piperaquine phosphate 320mg] và Artequick 2 thành phần [Artemisinine + Piperaquine phosphate], gần đây có Artequick hoặc Artequick-primaquine gồm 2 hoặc 3 thành phần [Artemisinie 40mg + Piperaquine 188mg + primaquine 2mg], Coarsucam hay Winthrop [Artesunate + Amodiaquine]. Hiện tại hơn 72 quốc gia trên thế giới theo khuyến cáo dùng thuốc ACTs như lựa chọn ưu tiên để điều trị P.falciparum.

A study of the impact of artesunate/SP use o­n oocyst/sporozoite carriage in mosquitoes. It is not unexpected that in an area of intense malaria transmission, with presumed large numbers of asymptomatic gametocyte carriers, that ACT does not result in a marked reduction in transmission indices. Whilst the result in not unexpected the methodology is thorough and is likely to be useful to others.

The authors discuss the protection conferred in the postpartum by IPTp or chemoprophylaxis, and the risk of recrudescence of resistant parasites due to inadequate treatment in the gestational period.

In a study of drug quality in Ghana, surprisingly, quality was found to be uniformly high ( of 33 drugs collected from 16 different shops, o­nly o­ne contained less than 80% of its purported active ingredient), and thus no significant relationship between price, treatment-seeking behaviour and the content of the active ingredients was observed. However, artemisinin monotherapy, which the WHO considers inappropriate therapy, was still widely available in Ghana in 2010.
