West-Highlanhs is still a "hot-spot" of malaria.
Report on the result of the survey of private clinics in the activity: "Pilot model on management and making statistical report of malaria cases" in Pleiku City, Gia Lai province

I. Background

In recent years, the effort of malaria control of Central Vietnam - West Highlands contributed to decrease the rate of malaria morbidity and mortality, no malaria outbreak occurred. The malaria control program achieved some of targets of national strategy o­n malaria control and elimination in Vietnam in the period of 2011-2020 and orientation to 2030.

However, to collect the indexes for the evaluation in the period of malaria elimination, we need to manage the number of malaria cases and make a report o­n the cases, including at public health services and private clinics. But the contribution of private clinics o­n the management and report of malaria cases has been still limited, so the private clinics play important role in malaria elimination strategy.

In order to deploy the activity "Pilot model o­n management and making statistical report o­n malaria cases of private clinics in the provinces of Gia Lai and Dak Lak", the IMPE-Quy Nhon appointed the research mission to go to Gia Lai province as follows:

IV. The content of the research mission

1. The content

- To survey 30 selected private clinics

- To guide to make report o­n statistics of malaria cases according to the circular No.54 of the Ministry of Health (MoH).

- To guide to use rapid diagnosis tests to detect malaria cases.

- To guide to detect and treat malaria cases according to the guidance of MoH.

2. The content of the survey:

2.1. General information of 30 private clinics

        Through the survey of 30 selected private clinics, there was 1 private general hospitals, making up 3.33%, 2 private general clinics (6.67%) and 17 internal medicine clinics (56.66%). The results were as follows:

- All of the health services in the survey had work permit (100%).

- All of heads of the clinics were doctors, with 21 men (70%) and 9 women (30%); and their average age was 50.3, with the oldest 67 and the youngest 32.

- The average working time of the health services was 7.5 years. The longest health service was the internal medicine clinic of Dr. Kim Xoa in 27 Tran Phu street, Pleiku city, Gia Lai, which was established in 1984; and 4 internal medicine clinics was established at the first year of 2016.

- The total of staff of 30 health services included 371, with 112 doctors (30.20%) and other staff. Hoang Anh Gia Lai General Hospital had the largest number of staff, with 240 staff (64.7%), among them 50 doctors (44.6% of total of doctors).

- In the survey of 30 health services, there was o­nly Hoang Anh Gia Lai General Hospital having antimalarial drugs such as ACT, Chloroquin, Primaquin, making up 3.33% compared with that other 29 clinics which there was no antimalarial drugs (making up 96.67%)

- To examination technology for malaria parasites, there was 1 clinic having examination technology for malaria parasites by microscopes or rapid diagnostic tests, making up 3.33%; 6 clinics having microscopes but no rapid diadnostic tests; 23 clinics having no examination technology for malaria parasites, making up 76.6%.

- O­ne clinic had guidelines for the diagnosis and treatment of malaria 2013 according to the Ministry of Health, making up 3.33% compared with that the total of clinics, 29 clinics had no the guidelines, making up 96.67%.

- The clinics have not been trained o­n diagnosis and treatment of malaria, making up 100%.

- O­ne clinic had statistic reports o­n malaria patients, making up 3.33%; the Hoang Anh Gia Lai general hospital sent reports to the city health centre. The remaining health services are the internal medicine clinics, so they had no any statistical report o­n malaria patients, making up 96.67%.

- 100% of clinics suggested mode of report in deploying the model through electronic mailbox and making report according to the circular No.54 of MoH.
