Workshop on Review of Project Implementation in the period of 2009-2013 and launching 2014 Workplan of the Viet Nam Global Fund Malaria Control Project

            On 25th February 2014, in Hoi An City, Quang Nam Province, the project management unit (PMU) of Vietnam Global Fund Malaria Control Project organised "Workshop o­n Review of Project Implementation in the period of 2009-2013 and launching 2014 Workplan of the Viet Nam Global Fund Malaria Control Project".

 

Review of VGF Malaria Control Project Implementation in the period of 2009-2013

The VGF Malaria Project "Strengthening the community-based malaria control activities in high-risk populations, and enhancing the capability and sustainability of the National Malaria Control Programme" was carried out within 5 years from 2009 to 2013. The general aims of the project are "to strengthen community-based malaria control activities in high-risk populations (especially mobilized population groups), to enhance surveillance activities in high-risk projected provinces, and to enhance the capability and sustainability of the National Malaria Control Programme, including specific objectives as follows:

           Objective 1: Enhance the access to and use of malaria control measures effectively;

            Objective 2: Enhance the access to and use of malaria diagnosis and treatment measures effectively;

            Objective 3: Enhance the surveillance and containment of malaria outbreaks;

            Objective 4: Enhance the capacity of the National Malaria Control Programme.

According to the report of the PMU of VGF project, most of malaria indexes in the period of 2009-2013 achieved or exceed the given target; especially the indexes of annual malaria morbidity and mortality rates decreased significantly. By 2013, there were 6 malaria deaths recorded in 6 provinces of the whole country. However, two indexes have not been achieved the given targets, including the rate of malaria parasites/population and the number of rapid diagnosis tests used to detect malaria cases in projected provinces.

Malaria deaths

From 2009-2013, there were totally 54 malaria deaths, and this index has tended to decrease from 2009 up to now. In 2013, there were 6 malaria deaths recorded in 6 provinces of the whole country, of which 4 cases were recorded in 4 projected provinces of Phu Yen, Khanh Hoa, Gia Lai and Binh Phuoc, decreasing by 80% as compared with 2008 (4/20). Especially, many cases of malaria mortality reported in Binh Phuoc and Gia Lai within 5 years with the total numbers were 13 and 8 respectively. No malaria deaths were found in the remain projected provinces from 2009 to 2013.

Malaria patients

In 2013, the number of malaria cases of 29 projected provinces reduced by 39.7% against 2008 (26,342/43,691). There were 16 provinces with decreased malaria morbidity rate by more than 50% as compared with 2008 included Lai Chau, Dien Bien, Son La, Lao Cai, Ha Giang, Bac Kan, Thanh Hoa, Nghe An, Quang Binh, Quang Tri, Thua Thien - Hue, Quang Ngai, Dak Nong, Tay Ninh, Bac Lieu and Ca Mau. 9 provinces with decreased malaria morbidity less 50% against 2008 included Cao Bang, Ha Tinh, Quang Nam. Khanh Hoa, Ninh Thuan, Binh Thuan, Kon Tum, Lam Dong, Binh Phuoc. 25/29 projected provinces had decreased rate of malaria morbidity, 4 provinces had increased rate of malaria morbidity as compared with 2008, including Phu Yen (+ 152.8%), Binh Dinh (+ 74.9%), Gia Lai (+ 79.2%), Dak Lak (+ 4.3%).

Malaria parasites

In 2013, the rate of malaria parasites infection of 29 projected provinces increased highly as compared with 2008 (+ 55.9%). 15 provinces had increased rate of malaria parasites, with more 100% rate such as Phu Yen (+342.3%), Gia Lai (+212%), Dak Lak (+192%), Binh Dinh (+179.7%), Kon Tum (+174.2%), Quang Ngai (152.9%), Quang Nam (117.2%). 13 provinces with decreased malaria parasites, included Lai Chau (-91.5%), Ha Giang (-90%), Dien Bien (-74.5%), Cao Bang (-69.6%), Tay Ninh (-63.4%). The provinces with increased rate of malaria parasites mainly concentrated in provinces of the central coastal area and the west highland. The provinces with decreased rate of malaria parasites were mainly in the North and the South. Especially, there were 4 provinces with increased rate of malaria morbidity and malaria parasites as compared with 2008, including Phu Yen, Binh Dinh, Gia Lai and Dak Lak.

           The cause of malaria parasite increase was mainly due to mobilised people and people doing seasonal works, going these areas to other o­nes. Besides, the utilisation of rapid diagnosis tests for malaria parasites at medical centres, especially at communal health stations contributed to detect infected rate of malaria parasites.

           The project achieved the significant results, indexes of malaria morbidity, malaria death decreased considerably compared with the given targets. The project concentrated o­n strengthening the ability of medical staff from centre to grass-root levels; at the same time, upgrading equipment for malaria control at grass-root levels. Moreover, the project attached special importance to enhance the ability for medical staff of institutes of malariolory, parasitology and entomology. The success of the project created favourable conditions to maintain the achieved results and continued to decrease rate of malaria morbidity and mortality, no malaria outbreaks.

Plan to deploy the Global Fund project to control malaria in 2014

In the years from 2014 to 2015, the Global Fund project to control malaria is going to be funded by the Global Fund to fight AIDS, TB and Malaria due to the Transitional Funding Mechanism in order to support some essential activities for maintaining the past results while the Global Fund hasn't provided new funding mechanism for the following period. This Project was approved, amended and supplemented more capital by the Government and the implementing time is extended to 2015.

The general objective of the Project

"To strengthen the community-based malaria control, focus o­n the remaining highly-endemic areas and the high risk group (particularly the mobile migration population), to enhance the supervision in the selected province with high risk of malaria and malaria epidemic resurgence, to reinforce the capacity and the sustainability of the National Malaria Control Program" with the specific objects:

Object 1: Maintain the approach and usage of malaria diagnose and treatment effectively
        Object 2: Maintain the approach and usage of malaria diagnose and treatment effectively
        Object 3: Maintain the supervision and control the malaria epidemic.
        Objective 4: Maintain the capacity of the National Malaria Control Program

Scope of activities

The project is going to be deployed in 1,902 communes in 156 districts of 29 provinces, including 89 districts and 940 communes of 14/15 provinces of the Central - Western Highland region (Quang Binh, Quang Tri, TT-Hue, Quang Nam, Quang Ngai, Binh Dinh, Phu Yen, Ninh Thuan, Binh Thuan, Gia Lai, Kon Tum, Dak Lak and Dak Nong)

Impact index (03 indexes)

1. Malaria morbidity/1,000 inhabitants/year: 0.47/1,000 inhabitants. 2. Malaria parasite morbidity per 1,000 inhabitants/year: <0.2/1,000 inhabitants. 3. Malaria mortality 100.000 inhabitants/years <0.001.

Result index of the Project (08 indexes)

1. Purchase and provide 638,450 long-lasting insecticide treated double bed-nets (LLIN) to the endemic areas and new migrants (region 4 and region 5). 2. Protect the group of inhabitants who usually till the fields, go into the forest and sleep in the mountain field with long-lasting insecticide treated single bed-nets: 200,000 single bed-nets. 3. Protect the seasonal employees in the malaria endemic regions (region 4 and region 5) with long-lasting insecticide treated bed-nets: 20,000 double bed-nets. 4. Perform la,/test o­n the people who are under suspicion of infecting malaria: 90%. 5. The number and proportion of malaria patients (infected with P.falciparum and P.vivax) are treated with front line anti-malarial drugs (ACTs for P.falciparum and CQ for P.vivax) according to MoH regulations: 100%. 6. The number and proportion of districts are monitored by the central and provincial authorities: 100%. 7. The number of medical stations reported adequately, punctually as regulated: 100%. 8. The proportion of districts and medical stations not lacking anti-malarial combination drugs and rapid diagnostic test in over 7 successive days in 6 months: 100%

Expenses

The funding of the Global Fund: 5,583,243 USD, of which 4,406,866.2 USD was allocated for LLIN purchase; 265,732 USD was allocated for rapid diagnostic test. The expense was met for the activities: monitor, communication to change the behaviors: 780,156 USD and the administration expenses is about 130,488.8 USD. The counterpart fund from the National Malaria Control and Elimination Project is 250,000,000 VND.

Hopefully, with the support of the Global Fund to control malaria, we would reinforce the capacity and the sustainability of the National Malaria Control Program as well as the effectively concentration of malaria control resources for people living in the highly endemic area and the high risk group.
