The survey of private health facilities in Dak Lak

1. The result of the survey of private clinics in the study

            The survey of 30 selected private health facilities and the analysis of the indexes show as follow:

- All of private hospitals, private clinics in the survey have work permit according to the regulation of the Ministry of Health (MoH), reaching 100% (30/30).

- The system of private hospitals and clinics are as follow:

Remark: There are 2 private general hospitals, making up 6.67% in total, 2 private paediatric clinics (6.67%), 9 private general clinics (30%) and 17 private internal medicine clinics (56.66%).

- The education level, sex and average age of 30 heads of private health facilities in the model.

Remark: All of heads of private health facilities have the standard of doctor. There are 25 men (83.33% and 5 women (16.67%); the average age is 53, among them the oldest person is 79 years old, the youngest person is 43 years old.

- The average working time of the health facilities is 8.33 years. The longest health facility is the internal medicine clinic of Dr. Lan in No.2 Village, Thuan Hoa commune, which was established in 1992; the newest heath facility is the internal medicine clinic of Dr. Ykrec Knul at 33 Tran Phu street, which was established at the first year of 2016.

- The number of doctors, nurses and staff are in health facilities in the model

Remark: The number of staff of 30 private health facilities are 874, with 330 doctors (37.76%) and other staff. Thien Hanh General Hospital has the largest number of staff, with 349 (39.93%), among them 133 doctors (40.30% of total of doctors)

- In the survey of 30 health services, there is o­nly Thien Hanh General Hospital having antimalarial drugs such as ACT, Chloroquin, Primaquin, making up 3.33% in comparation with that other 29 clinics which there are no antimalarial drugs (making up 96.67%)

- To examination technology for malaria parasites, the results show as follows:

Remark: In 30 private health facilities of the model, there were 13 clinics having examination technology for malaria parasites by microscopes or rapid diagnostic tests, making up 43.33%, with 13 clinics having microscopes and 9 clinic having microscopes and rapid diadnostic tests. 17 clinics had no examination technology for malaria parasites, making up 56.67%.

- 12 private health facilities have malaria diagnosis and treatment guidelines according to the 2013 Ministry of Health's therapy, accounting for 40% of the total surveyed facilities, 18 private health facilities don't have guideline, accounting for 60%.

- 7 private health facilities attended a training course o­n malaria diagnosis and treatment, accounting for 23.33%, 23 facilities haven't been trained, accounting for 76.67%.

- 5 private health facilities have reports o­n malaria cases, accounting for 16.67%, among of them, Thien Hanh General Hospital reported to Dak Lak provincial Malaria Control Centre, 4 remaining facilities reported to the city's Health Center, 25 facilities didn't have reports o­n malaria cases, accounting for 83.33% of the total 30 surveyed facilities.

- 100% facilities proposed the method of reporting through the email model and reporting according to the 54 circular of the Ministry of Health.

2. Results of the malaria case reports and management of the private health facilities.

2.1. Malaria case report, weekly report, monthly report situations of the facilities.

Remark: Of the 30 facilities that participated in the model from July to December 2016, 22 facilities have weekly and monthly reports, accounting for 73.33%. And 8 facilities din't have weekly and monthly reports, accounting for 26.67%.

3. Conclusions

3.1. Survey o­n the private health facilities that participated in the model

- 30 private health facilities of Buon Ma Thuot city, Dak Lak province were surveyed.

- All of the private health facilities that participated in the malaria case management and statistics model agreed to join the model.

- All of the private health facilities chose to report through email and reporting according to the circular No.54 of the Ministry of Health.

3.2. Results of cases management and cases report of the facilities

- In the pilot time, 1 facility detected the malaria patient and treated and reported this case prior to 24h, accounting for 100%.

- Of the total 30 facilities that participated in the model, 22 facilities have weekly and monthly reports, accounting for 73.33%.

- Of the total 22 facilities that have reports, 19 facilities have weekly, monthly reports in accordance with the prescribed time, accounting for 86.36%.

- Of the 22 facilities that have reports, all of them reported weekly and monthly through email, accounting for 100%.

4. Reccommendations

- Maintain the cases reports, weekly reports, monthly reports in the private health facilities in order to manage malaria cases more closely;

- Provincial Department of Health should issue Decision o­n malaria case report for the local private health facilities and reporting due to the circular No.54 of the Ministry of Health.
