Report on the result of the survey of private clinics in the activity: "Pilot model on management and making statistical report of malaria cases" in Buon Ma Thuot city, Dak Lak province
I. Background In recent years, the effort of malaria control of Central Viet Nam-West Highlands contributed to decrease the rate of malaria morbidity and mortality, no malaria outbreak occurred. The malaria control program achieved some of targets of national strategy on malaria control and elimination in Viet Nam in the period of 2011-2020 and orientation to 2030. However, to collect the indexes for the evaluation in the period of malaria elimination, we need to manage the number of malaria cases and make a report on the cases, including at public health services and private clinics. But the contribution of private clinics on the management and report of malaria cases has been still limited, so the private clinics play important role in malaria elimination strategy. In order to deploy the activity "Pilot model on management and making statistical report on malaria cases of private clinics in the provinces of Gia Lai and Dak Lak", the IMPE-Quy Nhon appointed the research mission to go to Dak Lak province as follows: II. The content of the research mission: - To survey 30 selected private clinics; - To guide to make report on statistics of malaria cases according to the circular No.54 of the Ministry of Health (MoH); - To guide to use rapid diagnosis tests to detect malaria cases; - To guide to detect and treat malaria cases according to the guidance of MoH. 2. The content of the survey 2.1. General information of 30 private clinics Through the survey of 30 selected private clinics, there were 2 private general hospitals, making up 6.67%, 2 private paediatric clinics (6.67%), 9 private general clinics (30%) and 17 internal medicine clinics (56.66%). The results were as follows: - All of the health services in the survey had work permit (100%); - All of heads of the clinics were doctors, with 25 men (83.33%) and 5 women (16.67%); and their average age was 53, with the oldest 79 and the youngest 43; - The average working time of the health services was 8.33 years. The longest health service was the internal medicine clinic of Dr. Lan in No.2 Village, Thuan Hoa commune, which was established in 1992; the newest heath services was the internal medicine clinic of Dr. Ykrec Knul at 33 Tran Phu district, which was established at the first year of 2016; - The total of staff of 30 health services included 874, with 330 doctors (37.76%) and other staff. Thien Hanh General Hospital had the largest number of staff, with 349 (39.93%), among them 133 doctors (40.30% of total of doctors); - In the survey of 30 health services, there was only Thien Hanh General Hospital having antimalarial drugs such as ACT, Chloroquin, Primaquin, making up 3.33% compare with that other 29 clinics which there was no antimalarial drugs (making up 96.67%); - To examination technology for malaria parasites, there were 13 clinics having examination technology for malaria parasites by microscopes or rapid diagnostic tests, making up 43.33%, with 13 clinics having microscopes and 9 clinic having microscopes and rapid diadnostic tests. 17 clinics had no examination technology for malaria parasites, making up 56.67%; - 12 clinics had guidelines for the diagnosis and treatment of malaria 2013 according to the Ministry of Health, making up 40% compared with that the total of clinics, 18 clinics had no the guidelines, making up 60%; - 7 clinics were trained on diagnosis and treatment of malaria, making up 23.33%; 23 clinics had not been trained, making up 76.67%; - 5 clinics had statistic reports on malaria patients, making up 16.67%; the Thien Hanh general hospital sent reports to the provincial centre of malariology, parasitology, entomology of Dak Lak, the remaining clinics sent reports to the city health centre. 25 clinics had no any reports on malaria patients, making up 83.33%; - 100% of clinics suggested mode of report in deploying the model through electronic mailbox and making report according to the circular No.54 of MoH.
